Users can find this modal when visiting the Shapeshift web application. What works well here is that they explain what they would collect, why, and what they would not. This is an excellent example of being transparent and giving control to the users.
Though one thing I want to point out here is that the primary CTA is more in favour of Shapeshift's preferences by tricking users into clicking on it, and the options that allow users to reject are treated as a secondary button made specifically so the user will miss it. This could count as a dark pattern as well.
